Logická hra podobná Tetrisu, Dr. Mario představuje maskota Nintenda Maria jako doktora. Hratelnost spočívá v pouštění oboustranných vitamínových kapslí do hracího pole o šířce 8 bloků a výšce 16 bloků, připomínajícího lahvičku od léků, které je osídleno viry tří barev.
